Students must confirm their insurance, immunization, and emergency contact information. This process can be completed on MyOC. For these instructions, you must first be logged in. For more information, please see our instructions on how to Log into MyOC.
Locate the Form
- Click on the myOC tab and then click Services
- Search for Insurance
- Click on Insurance, Immunization information, and Emergency Contacts update under Enrollment Information.
Fill Out the Form
Press submit to begin the process. You will need the following information:
- Insurance Information
- Emergency Contact Information
- Immunization Records
- List of Current Prescriptions
